After submitting a comprehensive online intake questionnaire via Charm EHR, you will be ready for your first Zoom session with Dr. Amber Jade. The first 75 minutes together will be spent discussing your life history, past medical record, present health concerns, goals for the future, and any questions that arise. Your Personal Wellness Plan will then be presented to you and reviewed in detail. Now you must tap into the healing power within to truly bring your health and wellness goals into reality! Move at your own pace and be kind to your Self. A follow up consult is recommended every 4-8 weeks for 6 months-1 year to continue optimally striving for your goals.
